• Signed signature form from the Application portal (or application form) *
• Two new/recent passport size photos with white background *
• Filled-in Power of attorney form
• Questionnaire to be filled out by the applicant *
• A print-out of this list of the documents you have to hand in, filled out and signed by you *

If missing documents

• If you are missing any of the documents, a filled out form about missing documents

• Passport – copy of all used pages *
• Birth certificate *

• Copy of the reference person’s passport – all used pages *
• Description of the child’s situation

If only one parent

• Documentation from an official authority stating who has parental responsibility for the child *
• If the parents have joint parental responsibility: a consent form which has been filled in by the other parent
• If the parents have joint parental responsibility: a copy of the passport or other valid identification document for the parent who does not live in or is not moving to Norway
• If the other parent is dead: death certificate
• A written statement explaining the applicant’s care situation in the country of residence, with information about who now cares for the child *

If the reference person works

• Contract of employment or a confirmation from the employer *

If the reference person is self-employed

• Tax return with all the necessary supporting documents (income statement etc.) *
• If you have an accountant: Confirmation from your accountant showing the profits of your company after the last tax year

Financial guarantee from spouse/cohabitant

• Guarantee form for residence in a family immigration case *
• Contract of employment or a confirmation from the employer *
• The latest tax assessment notice *
• The three most recent payslips *
