Parent of Norwegian child under 18

• Signed signature form from the Application portal (or application form) *
• A new passport photo with a white background *
• Filled-in Power of attorney form
• UDI's checklist, filled out and signed (this list) *

• Passport and copy of all used pages in the passport *

If missing documents

• If relevant, a written explanation as to why you cannot present one or more of the documents mentioned above

• Copy of the child’s passport – all used pages *
• Birth certificate for the child *
• Documentation which shows that you have a share in the parental responsibility for the child *
• Documentation which shows that the parents of the child are not married to each other *
• Information about who the child is currently living with *
• Description of the child’s situation (optional)

If missing documents

• If relevant, a written explanation as to why you cannot present one or more of the documents mentioned above
